  Please follow below steps to download application form
  
  1. Fill all fields start by select the type of form you want to download and click download button
  
  2. Right click on form and click save
  
  3.Make the payments of(30,000/=) at any NMB Branch/Tigo Pesa/Mpesa/Air tel Money/Hallo Pesa.
  
  4.Use Referenec number shown on the TOP of the form(SAS005XXXXXXXX) while you make the payment as "Namba ya Kumbukumbu" and 888999 as "Namba ya Kampuni".
